All athletes must complete an Athletic Packet prior to the 1st day of practice.  

Athletic Participation Information

To participate in Athletics, you must maintain a 2.0 GPA with no F's. You must be in good disciplinary standing at school.

All student athletes must complete a new online registration each year, including uploading a current signed physical form by a physician dated after June 1, 2026.


Follow the steps below to register for 7th and 8th grade Harper Athletics:

  1. You can find the following forms at https://harper.djusd.net/athletics

  2. Before going to the doctor, print out a Sports Physical Form (English) (Spanish) for your doctor to sign. Parents must sign the physical form as well. Hard copies of this form are also available in the Harper office.

  3. After completing the Sports Physical, complete FinalForms Athletic Paperwork

  4. Read through all the documents and complete all required fields for both parents and students. Please remember to fill in at least two emergency contacts, parent/guardian and other (grandparent, family friend).

  5. Upload a scan or clear photo of your athlete’s current physical to FinalForms online and keep the original copy for your records.

  6. All data will be electronically filed with the Harper Athletic Director.

  7. You will receive an email when your Athlete is cleared to play.

Please note: Students cannot participate in practices or games until the FinalForms Athletic Packet and the Sports Physical is completed. Please see below for the start dates for the 2026-2027 Sports Schedules. Students only need to complete the online registration one time during the school year, but they should complete the sign-ups for each sport. Information to sign up for each sport will be in the bulletin and sent out by the Athletic Director, Darcie Hauger,  and sent to all student/parents’ emails.

Harper Athletics requests a $25.00 donation per athlete per sport. These donations go towards uniforms, equipment, and other supplies necessary for our sports teams. Thank you for your support. Donations be made through the Harper WEBStore (link on the Athletics web page) or in the front office.
